Claims procedures relative to medical and dental benefits

  • Written details of all claims should be sent to the claims administrator as soon as possible and in any event not later than 365 days from the beginning of the medical treatment.
  • All documentation relating to the claim including the claim form and accounts are originals - faxed and scanned copies are permissible up to $9,000 CAD.
  • The required premiums must have been paid relative to the insured person making the claim.
 

24 Hour Emergency Service

In the event of accident or illness resulting in hospitalization, the claims service provider must be contacted immediately following such hospitalization.

Have the following information available:

  • Certificate Number and Contract Number as shown in the schedule.
  • Insured Persons name and address abroad
  • Telephone number from which you are calling and can be reached at
  • Name and telephone number of the Doctor and Hospital attending you.
  • Nature of the emergency.

Failure to contact the Emergency Service Company and obtain authorisation may prejudice the claim and mean that not all the costs involved will be paid. The Assured/Insured Person should not attempt to find their own solution and then expect full reimbursement from the Underwriters,
